This plan is no way to be considered as “pet health insurance” or “indemnity property insurance.”

Pet Care Plans may be paid in full at time of registration, or paid by monthly payments. If you pay the plan in full, you will receive a 10% discount.

This plan covers all services listed and only those services. It does not cover:
• Any services provided by outside veterinarians
• Any fees for services recommended as a result of illness or accidental injury
• Any fees for services not listed in the plan

Payments will be auto-deducted from the Client’s credit or debit card. An overdraft and reprocessing fee of $25 will be charged for any rejected charges.

Pet Care Plans are not transferable and apply only to the patient identified at the time of enrollment for as long as the pet is owned by the same owner. Services are good at CAH only.

Pet Care Plans must be used within 365 days (one year) of purchase date. This agreement will automatically renew 12 months from the date this contract is signed unless canceled by either party with a 30-day notice. Puppy and Kitten plans will be renewed as adult plans at the same respective level and monthly fee.

Pet Care Plan levels may be upgraded at any time while under contract by notifying CAH. Plan upgrades will require payment of the monthly cost difference from the beginning of the contract year to the date of the upgrade.

If the Client cancels before any services are rendered, the entire amount paid for the current year will be refunded.

If the Client cancels after any or all services are rendered, the Client will be liable to CAH for remaining installments for the year or the full amount of the standard price of services already rendered.

In the event of unforeseen circumstances, such as a pet’s death or the Client moves, the Client will pay the balance of payments or the standard price for services rendered, whichever is less.

There are no refunds from any prior year, for any reason.

The Client is responsible for notifying CAH if there is a change in the Client’s credit/debit card account.

If the Client fails to pay any installment within 30 days of the due date, CAH may immediately terminate the agreement. After such termination, CAH retains all paid monthly payments. Additionally, the Client will be liable to CAH for the difference between the standard price of services already rendered and all paid monthly payments.

CAH will withdraw from my debit or credit card account the payment listed above on a monthly basis. For the plans that are paid in full, CAH will automatically withdraw the total amount of the plan on the anniversary date of the renewal. I (the client), will be notified that the renewal date is approaching, but such withdrawals will continue until the total amount owed to CAH is paid in full or until the recurring draft has been stopped by CAH or by me. Debits to my account may vary from the day I selected due to weekends, holidays, or the number of days in certain months such as February.

CAH reserves the right to adjust monthly fees on any enrollment anniversary date and/or cease to provide the plans at any time.
